Steady@Home Program

At Project Health, we believe one fall can change your life , and it’s not the change you want. That’s why we created our Steady @ Home Program, an initiative focused on helping older adults reduce fall risks, build strength, and stay independent, all in the comfort and safety of home.

Program Highlights

  • In-home assessment

    A licensed nurse and/or physical therapist visits your home to perform a comprehensive evaluation — no travel required.

  • Medicare may cover if conditions and eligibility requirements are met.

  • Balance & strength training

    CDC-recommended exercise routines guided by our in-home physical therapists to build stability, strength, and confidence. Our in-home nurse reviews medications and health concerns to prevent complications and falls.

  • Step-by-step mobility practice

    Personalized sessions to improve walking, balance, and mobility — helping you move with greater freedom and control in your own environment.

  • Family involvement & education

    We include family members and caregivers at every step — giving them the tools and knowledge to support your progress and encourage lasting results.
